Your safety is always our priority.

In preparation for your upcoming colour service, we’ll ask you to complete our Allergy Alert Testing Questionnaire.

You will receive this form by email 4 days before your colour appointment.
It must be completed before your visit and if your answers indicate that a skin test is required and you have not had one, we will not be able to carry out your colour service in line with current regulations. In this situation, you may still be charged for the appointment.

Our policy has been designed specifically for HEIR Salon and Education and follows current manufacturer guidelines as well as our insurance requirements.
As these may change, it’s important you read this information before every colour appointment to avoid your appointment being cancelled for health and safety reasons.


When a Skin Test is Required at HEIR Salon

You will need a skin test:

  • If you are a new client

  • If it has been more than 6 months since your last colour service or skin test at HEIR

  • If you are having a different brand of colour applied than usual

  • If any of your answers below indicate that it’s necessary


Your Allergy Alert Testing Questions

  1. Are you under 16 years of age?

  2. Have you had allergic reactions to hair colourants or skin tests before?

  3. Have you started any new medication? Please include prescription or over-the-counter medication, hormone therapy, acne medication, weight loss injections or other injectables.

  4. Have you had any significant medical changes since your last colour service?

  5. Have you had an allergic reaction to any type of skin tattoo including henna or semi-permanent makeup?

  6. Do you have any skin or face rashes, sensitive, irritated or damaged scalp?

  7. Are you new to HEIR Salon or having a different brand of colour applied than usual?

  8. Is it more than 6 months since you last had a colour at HEIR Salon or a skin test at HEIR Salon?

  9. Have you coloured your hair (including home toners) or had a colour at a different salon since your last colour at HEIR?

  10. Have you had an allergic reaction to anything since your last colour service?

  11. Have you had any type of henna, tattoo or semi-permanent makeup since your last colour service?
